Transition Plus

Preparing students with skills that promote a successful transition from high school to adult independent living. 

Transition Plus is a joint program with Hopkins and St. Louis Park school districts that works with young adults with disabilities, ages 18 to 21.

Please note: the main office for Transition Plus is located at 6715 Minnetonka Blvd. in St. Louis Park.

Transition Plus specializes in the areas of transition that prepare students for independent life:

  • Jobs and job training
  • Recreation and leisure
  • Community participation
  • Independent living
  • Post-secondary/post-school planning

Using the Individual Education Plan (IEP) team process, Transition Plus services help students achieve their goals by providing specialized instruction, training, and support. Students have access to skill-development activities designed to meet their individual needs in the five transition areas listed above.
